The Missing Referral Source report lists all patients who did not specify a referral source in their intake form.
This report includes the following columns, all of which can be filtered:
Patient: the patient’s name
First IE Date: the date of the patient’s scheduled initial evaluation
First IE Clinic: the practice location of the patient’s scheduled initial evaluation
This report can be exported as a CSV file by clicking the ‘download’ icon in the top right corner.
