This report displays each patient’s referral source per case, based on the response to “How did you hear about us?” in the patient intake form. If a patient has multiple cases, they may populate in this report more than once.
This report includes the following columns:
Patient: the patient’s name
Case: the case associated with the intake
First IE Date: the date of the scheduled initial evaluation for the applicable case
First IE Clinic: the clinic location of the scheduled initial evaluation
Source: the referral source selected in the intake
Source Details: additional details added in the intake (if applicable)
# Appts Scheduled: the number of appointments scheduled in the case
% Checked-In: the percentage of scheduled appointments that have been checked in
This report can be downloaded as a CSV file by clicking the ‘download’ icon in the top right corner.
