This report lists every patient created in Stride, regardless of active or discharged status.
The Client List report includes the following columns, all of which can be filtered:
Patient: patient name
Date of Birth: the patient’s date of birth
Birthday: the patient’s birthday (month/day)
Last Visit Date: the date of the patient’s most recent checked-in appointment
Last Visit Location: the practice location of the patient’s most recent checked-in appointment
Insurances: the insurance(s) listed in the patient’s chart
Referring Providers: the referring provider(s) listed in the patient’s chart
Status: active, discharged, new, never seen, or on hold
Email: the email address listed in the patient’s demographics
Phone Number: the preferred phone number listed in the patient’s demographics
Address 1: the address listed as Address 1 in the patient’s demographics
Address 2: the address listed as Address 2 in the patient’s demographics
City: the city listed in the patient’s address
State: the state listed in the patient’s address
Zip: the zip code listed in the patient’s address
This report can be exported as a CSV file by clicking the ‘download’ icon in the top right corner.
*Upon export, First Name and Last Name will separate into two columns for additional filtering and sorting.
