Intake forms will be automatically sent to the patient once the patient’s initial evaluation is 7 days away or it will be sent immediately if the appointment is within 7 days.
You can manually send an intake form by going to the patient’s case and clicking send.
In order for the patient to receive the intake form, the information below is required:
- Patient’s email address or mobile number
- Case
- A future initial evaluation appointment
If only a mobile number is on the patient’s chart the intake will be texted. If an email or both email and mobile number are on the patient’s chart then the intake will be emailed.
