DISCHARGE NOTES:
There are two types of Discharge notes available within Stride: Discharge and Quick Discharge.
The 'Discharge' note type is utilized for discharging a patient at a session.
The 'Quick Discharge' is utilized for discharging No visit, or when the patient is NOT being seen.
To initiate a discharge note, start a note in the same way you would at all other times and select one of the two discharge note types.
A Discharge Note looks similar to an Initial Eval or Progress Note, with the appropriate changes that allow you to discharge the case and indicate the discharge to the referring provider and payer.
A Quick Discharge is a much shorter discharge summary. See below:
DISCHARGE A CASE:
Cases will be discharged once a discharge note is locked.
Cases can be manually discharged by clicking on the case’s 3-dot more menu on the main level chart view. This will move the case to the “Discharged Cases” tab.
A discharged case can be re-opened by clicking on the same 3-dot menu.
