How to Create/Modify Intakes for your Practice:
Stride includes a feature to populate the IE from a patient’s previous intake, and here is a video demonstration of this feature: https://www.loom.com/share/619248ba311a41b1b33af84b2f94ba71
To complement this feature, you also have an option to create other intakes, such as a Returning Patient Intake. The Returning Patient intake would remove certain sections and/or be much skinnier for the patient's medical history so those details can come from the previous intake. To do this, you can go here:

Then, click the 3 dots to the right of "intake" and select "copy," then rename to your liking such as "returning patient intake."
Note: You can also modify your existing intake by simply selecting "edit" and then removing sections you no longer want visible for your patients.
After renaming, you will be able to remove sections from the new intake to make it shorter by deselecting checkboxes:
Some sections to consider turning off include the following:
- Patient Information
- Contact Information
- Insurance Information
- Medical & Societal History
- Medications
- Precautions
Once saved, this option will be available when scheduling moving forward. This will show up when you create a new patient, you will be able to select which intake you would like to send. You will also be able to select which intake to send when creating a new case for a current patient, as shown below in some examples:
