In Stride, once a note is locked, it cannot be deleted. A locked note is considered a permanent medical record that stays within the patient chart. However, Stride does have an "Amend Note" feature where you can make updates to the note itself, the charges billed, or both.
To create an amendment, select the three dots to the right of a locked note:
About 1 minute after a note is locked, the ability to Amend a note will be available to you:
Once you click "Amend," you will be able to add in any changes you wish to add and then proceed with clicking the lock button to lock your note. From here, you have TWO OPTIONS
Option 1: Proceed with creating a new claim for the amendment. Stride will create a new claim for every locked note unless the "Don't generate claim" option is selected. If you have made edits to the units billed in your amendment, you would proceed with creating a new claim at the sign-off process as shown below:
Option 2: Proceed with adding in other information for the amendment, but do not create a new claim. To avoid creating a new claim, you must select the "Don't generate claim" button at sign-off. This option would be applicable if you only made changes to the note itself, but did not make any changes to the procedure codes that were billed. Checking the box as shown below will not create a second claim for the same date of service:
Once you have successfully chosen your options, your amendment will be finalized in Stride. The original will still be visible and can be viewed as it is still considered a medical record for the patient.
