For Existing Patients: Transitional Progress Notes will be completed as the first note in Stride during your Clinical Launch week.
Transitional Progress Notes are a special note type utilized to pull together the POC data not available from your previous EMR including:
1. ICD-10 codes
2. Goals
3. POC frequency and duration
4. Procedures
This note type also resets the progress note count and allows for appropriate Progress Note tracking within Stride.
Ex) The patient had an established POC of 2 visits per week for 6 weeks, for a total of 12 visits, in your previous EMR. The patient has used 6 of those 12 visits.
- On the Transitional PGN in Stride, you would record 2 visits per week for 3 weeks in the POC frequency and duration section of the note, to properly transfer the POC information into Stride.
Remember:
- This is a billable note, not an extra note.
- At Signing, the "Mark as POC" box should be checked on this note (as shown in the video below).
- Transitional Progress Notes can be initiated once appointments are scheduled by your admin team.
- Key Phrases and Prefilled Templates can be created in advance of your Clinical Launch for your Assessment and Goals sections.
- If your practice is using Comprehend AI, you can dictate any additional information quickly & easily into the note ahead of the patient's visit.
