In Stride, the user can choose to change their note type at any time, as long as the note has not been locked yet by clicking the Change Note Type Button:
If you are upgrading your note type, you will not lose any data already entered in the note. If you are downgrading your note type, you may lose information because the downgraded note type may not have that section available to pull data into. An example would be going from an IE to a daily note, where a daily note does not have a POC field in the base template. Stride will always provide a reminder message explaining this prior to the user finalizing their note change:
Once the process is complete, you will see the note type change in the upper left hand corner of the note:

Note:
If you attempt to change a note to be an Initial Evaluation but you don't see the option available, verify the following:
- Make sure there is no other note in the case marked as IE
- If there is an IE appointment type, this will also prevent you from selecting IE when changing a note type. Update the appointment to be a non-IE appointment type, then you can change your note type. After changing the note type, you can update the appointment to be its original type.
