2. Transitional Progress Note: A Clinician's First Note In Stride

For Existing Patients: Transitional Progress Notes will be completed as the first note in Stride

Transitional Progress Notes are a special note type utilized to pull together the POC data not available from your previous EMR including: 

1. ICD-10 codes

2. Goals

3. POC frequency and duration

4. Procedures

This note type also resets the progress note count and allows for appropriate Progress Note tracking within Stride.

Remember:

• This is a billable note, not an extra note.
• Transitional Progress Notes can be initiated once appointments are scheduled by your admin team.
