In Stride, the following 3 options exist in the Note Finalization Process:
- Sign the note
- Request Cosign
- Request Signature
NOTE: These options may be different based on your organization's configurations.
Clinicians: If you have questions regarding the options available to you in the Note Finalization Process, please ask your Site Administrator.

1. Sign the Note
Selecting Sign the note & LOCK NOTE will complete the note finalization process. Use this if you are the Clinician who provided services, and no additional signature is required for any reason.
- As only one clinician is involved in the Note Finalization Process, this clinician will receive Credit in the PT Billing Report.
2. Request Cosign
Request Cosign is appropriate for PTA's, COTA's, and SLPA's. You will be asked to select the Rendering Clinician from a drop down menu that you wish to cosign the note. When using the Request Cosign feature the following are true:
- Both clinicians will populate on the PDF version of the note, along with their signatures.
- The Rendering Clinician will populate as the Rendering Provider on the claim.
- The Clinician Requesting Cosignature will receive credit in the PT Billing Report.
Note: For Eval Note types (PN, RE, Transitional PN) PTA's, COTA's, and SLPA's will only have the Request Signature Option (below), not Request Cosign.
3. Request Signature
If using the Request Signature, you will be asked to select the Rendering Clinician from a drop down menu that you wish to sign the note. When using the Request Signature feature, the following are true:
- Only the Rendering clinician will populate on the PDF version of the Note, along with their signature.
- The Rendering Clinician will populate as the Rendering Provider on the claim.
- The Rendering Clinician will receive Credit in the PT Billing Report.
- Student Clinicians in Stride have the Request Signature option as the ONLY option in the Note Finalization Process:
