This report tracks the status of patient intakes across all cases created in Stride.
This report includes the following columns, all of which can be filtered:
Patient: the patient the intake was sent to
Case: the case associated with the intake
IE Date: the scheduled date of the Initial Evaluation
Location: the clinic location associated with the Initial Evaluation
Status: Scheduled, Not Started, Patient Started, Needs Review, Reviewed, No Contact Info
Date Submitted: the date the intake was submitted by the patient
Upcoming Send Date: the date the intake is scheduled to send to the patient
Last Date Sent: the date the intake was most recently sent to the patient
Attempts: how many times the intake has been sent to the patient
Attempt to: the phone number or email address then intake was sent to
This report can be exported as a CSV file by clicking the 'download' icon in the top right corner.
