The POC report lists every note that has been marked as a POC. This report is helpful to track which plan of care notes have been sent to the referring provider and if they have been signed.
Notes can be marked as POC in two ways:
- Upon signing the note, check the box to the left of Mark as POC in the bottom right corner of the Note Finalization pop-up
- Within the case, click the three-dot menu to the right of the note and select Mark as POC
These POC notes can be faxed to the referring provider in two places:
- Upon signing the note using the eFax process: Note Finalization: Auto E-Fax
- With the case, click the three-dot menu to the right of the note and select eFax
This report includes the following columns, all of which can be filtered:
Patient: the patient associated with the POC note
Case: the case associated with the POC note
Primary Insurance: the primary insurance on the applicable case
Appointment Location: the clinic location of the visit associated with the POC note
Visit Date: the date of the visit associated with the POC note
Clinician: the clinician associated with the POC note
Signed Status: if the note has been signed by the MD (signed or unsigned)
Signed Uploaded Date: the date the signed POC was uploaded to the patient’s case
Sent Status: if the note has been sent to the MD (sent or unsent)
Last Sent Date: the date the POC was most recently sent to the MD
Note: A note may appear in this report multiple times if an amendment was made and both the original version and amended version(s) were sent to the MD.
Additionally, a note will reflect as "unsigned" if it was sent to a fax number that differs from the fax number listed for the referring provider in Stride.
This report can be exported as a CSV file by clicking the ‘download’ icon in the top right corner.
Important Note: If the patient's POC is an Initial Evaluation and there a physician script on file, Stride will automatically mark the status as 'Signed'. For more information on this, please reference this article: POC Compliance - 2025 Update
